Security Prescription Forms Reported Stolen or Fraudulent
The California State Board of Pharmacy advises that the following prescribers have reported that fraudulent California security forms for controlled substances have appeared in California pharmacies on forms where they are listed as prescribers. Because at least some prescriptions may not be legitimate prescriptions of the prescribers, the board is advising pharmacies that when filling a prescription written by one of these prescribers, please take appropriate precautions to verify the legitimacy of the prescription. This may include contacting the prescriber’s office using a phone number that has been obtained from a source other than the prescription form itself.
To report stolen or fraudulent security prescription forms, contact the boards complaint unit and provide the following information:
- Prescriber Name(s) and specialty as listed on the prescription forms (MD, DDS, OD etc.)
