Renew a Facility License
If you fail to renew your permit within 60 days of the expiration date, the permit will be cancelled pursuant to section 4402(e) of the Business and Professions Code. Once the permit has been canceled you must submit a new application (including all documents) and meet all requirements in effect at the time of reapplication.
Operating the premises without an active license is unlicensed activity, for which the owners or responsible parties may be cited and fined up to $2,500.

Automated Drug Delivery System
An automated drug delivery system (ADD), automated patient dispensing system (ADC) in a 340B Clinic/Medical Professional Practice, and an emergency medical services automated drug delivery system (ADE) license renewal application will be emailed directly to the primary license by the Board 60 days prior to the expiration date of the license, which is the same as the underlying primary license.
Licensure Information
- Relocation of an ADD, ADC, or ADE shall require a new application for licensure.
- Replacement of an ADD, ADC, or ADE shall require notification to the Board within 30 days.
- The primary license that holds an ADD, ADC, or ADE license shall notify the Board in writing within 30 days if use of the ADD, ADC, or ADE is discontinued.
- The ADD, ADC, or ADE license shall be canceled by operation of law if the underlying primary license is not current, valid and active.
- The ADD, ADC, or ADE license shall be renewed annually, and the renewal date shall be the same as the underlying primary license.
- The ADD, ADC, or ADE original license and current renewal license shall be displayed on the ADD, ADC, or ADE machine in a place where it may be clearly read by the public.
